Step 2

The application must be filled out in the prescribed format and signed by the applicant.

Requirement :

Application and must be accompanied by - certified true copies of

  • Basic medical degree(s)

  • Postgraduate qualifications(s)

  • List of Publications

  • Cheque/banker’s order for RM 1,000 for life members, RM 100 for ordinary membership and RM50 for associate membership

Step 3

Submit completed application form via post and email to:
